{"id":12692,"date":"2026-01-19T10:32:37","date_gmt":"2026-01-19T09:32:37","guid":{"rendered":"https:\/\/www.gerep.fr\/blog\/purchasing-power-and-mutual-insurance-real-gain-or-false-promise-for-employees\/"},"modified":"2026-06-18T14:40:41","modified_gmt":"2026-06-18T12:40:41","slug":"purchasing-power-and-mutual-insurance-real-gain-or-false-promise-for-employees","status":"publish","type":"post","link":"https:\/\/www.gerep.fr\/en\/blog\/purchasing-power-and-mutual-insurance-real-gain-or-false-promise-for-employees\/","title":{"rendered":"Purchasing power and mutual insurance: real gain or false promise for employees?"},"content":{"rendered":"\n\n\t<p>Lorsqu&#8217;on aborde la question du pouvoir d&#8217;achat dans le cadre de la mutuelle d&#8217;entreprise obligatoire, un paradoxe appara\u00eet rapidement : fait-elle vraiment gagner de l&#8217;argent au salari\u00e9, comme d&#8217;autres <a href=\"https:\/\/www.gerep.fr\/blog\/avantages-sociaux-ce-qu-il-faut-savoir\/\" target=\"_blank\" rel=\"noopener\">avantages sociaux<\/a>, ou en co\u00fbte-t-elle finalement davantage qu&#8217;on ne le pense ?<\/p>\n<p>&nbsp;<\/p>\n<h2>La mutuelle d&#8217;entreprise<\/h2>\n<h3>La mutuelle obligatoire : un dispositif qui p\u00e8se sur les budgets<\/h3>\n<p>Depuis 2016, la <a href=\"https:\/\/www.legifrance.gouv.fr\/jorf\/id\/JORFTEXT000031912641\" target=\"_blank\" rel=\"noopener\">loi ANI<\/a> impose \u00e0 toute entreprise du secteur priv\u00e9 de mettre en pace une mutuelle collective pour ses salari\u00e9s. Pr\u00e9sent\u00e9e comme un progr\u00e8s social, cette obligation soul\u00e8ve pourtant une question l\u00e9gitime : <strong>am\u00e9liore-t-elle v\u00e9ritablement le pouvoir d&#8217;achat des Fran\u00e7ais, ou repr\u00e9sente-t-elle finalement une charge suppl\u00e9mentaire d\u00e9guis\u00e9e ?<\/strong><\/p>\n<p>Les chiffres parlent d&#8217;eux-m\u00eames. Selon la Mutualit\u00e9 Fran\u00e7aise, les cotisations de mutuelles ont augment\u00e9 en moyenne de <strong>6 % en 2025<\/strong>, apr\u00e8s une hausse de 7,3 % pour les contrats collectifs en 2024. Pour un salari\u00e9, cela repr\u00e9sente entre 40 et 150 \u20ac par mois selon les garanties, soit 480 \u00e0 1 800 \u20ac par an directement pr\u00e9lev\u00e9s sur le salaire.<\/p>\n<p>Face \u00e0 une inflation d\u00e9j\u00e0 forte et des augmentations de salaire limit\u00e9es, cette charge p\u00e8se lourdement. Le S\u00e9nat fran\u00e7ais a d&#8217;ailleurs cr\u00e9\u00e9 en 2024 une mission d&#8217;information d\u00e9di\u00e9e \u00e0 l&#8217;impact des <a href=\"https:\/\/www.gerep.fr\/vos-besoins\/assurance-sante\/\" target=\"_blank\" rel=\"noopener\">compl\u00e9mentaires sant\u00e9<\/a> sur le pouvoir d&#8217;achat, signe que la question pr\u00e9occupe jusqu&#8217;aux plus hautes instances.<\/p>\n<p>&nbsp;<\/p>\n<h2>Les gains r\u00e9els pour le pouvoir d&#8217;achat<\/h2>\n<h3><strong>La participation patronale ou financement de l&#8217;employeur\u202f: un gain r\u00e9el&#8230; mais partiel<\/strong><\/h3>\n<p><strong>Premier point incontestable :<\/strong> l&#8217;employeur finance au minimum 50% de la cotisation pour la part obligatoire. Donc, sur une cotisation \u00e0 100\u20ac par mois, vous \u00e9conomisez imm\u00e9diatement 50\u20ac, soit <strong>600\u20ac par an<\/strong>. Certaines entreprises g\u00e9n\u00e9reuses vont m\u00eame jusqu&#8217;\u00e0 prendre en charge 60, 70, voire 100% des cotisations.<\/p>\n<p>C&#8217;est une aide directe, inscrite sur la fiche de paie, et que les salari\u00e9s applaudissent volontiers, qu&#8217;ils soient jeunes, c\u00e9libataires ou parents d&#8217;une famille nombreuse.<\/p>\n<h3><strong>Le tarif mutualis\u00e9 et l&#8217;acc\u00e8s \u00e0 une bonne couverture<\/strong><\/h3>\n<p>Gr\u00e2ce \u00e0 l&#8217;effet de groupe, une mutuelle collective co\u00fbte g\u00e9n\u00e9ralement moins cher qu&#8217;un contrat individuel \u00e0 garanties \u00e9quivalentes. Plus l&#8217;entreprise compte de salari\u00e9s, plus le risque est mutualis\u00e9 et les tarifs n\u00e9goci\u00e9s \u00e0 la baisse. Par ailleurs, le contrat collectif offre g\u00e9n\u00e9ralement :<\/p>\n<ul>\n<li>des garanties mieux structur\u00e9es<\/li>\n<li>un acc\u00e8s optimis\u00e9 \u00e0 l&#8217;optique et au dentaire gr\u00e2ce aux r\u00e9seaux de soins et au 100% Sant\u00e9<\/li>\n<li>une protection solide en hospitalisation<\/li>\n<li>des services additionnels\u202f: t\u00e9l\u00e9consultation, pack m\u00e9decine douce, cures thermales, programmes de pr\u00e9vention.<\/li>\n<\/ul>\n<p>Pour un salari\u00e9 ayant des besoins r\u00e9guliers en soins ou une famille avec enfants, ces \u00e9conomies sont particuli\u00e8rement perceptibles.<\/p>\n<ul>\n<li>de plus, les compl\u00e9ments peuvent g\u00e9n\u00e9rer de la valeur per\u00e7ue souvent tangible, soit, impacter positivement la perception de l&#8217;entreprise ainsi que votre marque employeur et in fine, am\u00e9liorer la <a href=\"https:\/\/www.gerep.fr\/blog\/sante-mentale\/\" target=\"_blank\" rel=\"noopener\">sant\u00e9 mentale<\/a> mais aussi la <a href=\"https:\/\/www.gerep.fr\/blog\/la-qvt-devient-la-qvct\/\" target=\"_blank\" rel=\"noopener\">QVCT<\/a> de vos salari\u00e9s.<\/li>\n<li>le vrai gain : celui qui se r\u00e9v\u00e8le&#8230; le jour du sinistre<\/li>\n<li><strong>et si la mutuelle n&#8217;\u00e9tait pas un avantage social comme les autres\u202f?<\/strong><\/li>\n<\/ul>\n<p>Contrairement au ticket-restaurant ou \u00e0 <a href=\"https:\/\/www.gerep.fr\/vos-besoins\/epargne-salariale\/interessement-et-participation\/\" target=\"_blank\" rel=\"noopener\">l&#8217;int\u00e9ressement<\/a> qui mettent de l&#8217;argent dans la poche des salari\u00e9s, la mutuelle fonctionne diff\u00e9remment. <strong>C&#8217;est avant tout une assurance :<\/strong> vous payez une prime mensuelle et le gain ne se mat\u00e9rialise qu&#8217;en cas de besoin, quand le risque survient\u202f: hospitalisation, chirurgie, soins dentaires, impr\u00e9vu de sant\u00e9 lourd ou co\u00fbteux&#8230;<\/p>\n<p>C&#8217;est pr\u00e9cis\u00e9ment au moment du besoin que la mutuelle cr\u00e9e du pouvoir d&#8217;achat : elle \u00e9vite des d\u00e9penses que beaucoup ne pourraient pas assumer.<\/p>\n<p>&nbsp;<\/p>\n<h2><strong>Le gain \u00ab\u202fsurestim\u00e9 \u00bb (revers de la m\u00e9daille)\u202f:<\/strong><\/h2>\n<h3>La fiscalisation de la part employeur<\/h3>\n<p>Premier revers majeur, depuis 2013, la part financ\u00e9e par l&#8217;employeur est imposable au titre de l&#8217;IR (imp\u00f4t sur le revenu). Ce qui diminue d&#8217;autant le gain per\u00e7u. Un salari\u00e9 paye ainsi plus d&#8217;imp\u00f4ts en raison de la participation patronale \u00e0 sa mutuelle obligatoire.<\/p>\n<p>C&#8217;est-\u00e0-dire que la participation de votre employeur \u00e0 votre mutuelle est consid\u00e9r\u00e9e comme un avantage en nature imposable. Si votre employeur finance 50 \u20ac par mois, cette somme s&#8217;ajoute \u00e0 votre revenu <strong>imposable<\/strong>. Donc, plus la participation patronale est g\u00e9n\u00e9reuse, plus l&#8217;imp\u00f4t augmente.<\/p>\n<h3><strong>Les taxes \u00e9lev\u00e9es sur la compl\u00e9mentaire sant\u00e9<\/strong><\/h3>\n<p>Les cotisations int\u00e8grent en moyenne plus de 20 % de taxes et contributions diverses. Ces montants n&#8217;am\u00e9liorent ni la prestation, ni le pouvoir d&#8217;achat du salari\u00e9. Ils constituent un surco\u00fbt structurel, par exemple :<\/p>\n<ul>\n<li>la <a href=\"https:\/\/www.urssaf.fr\/files\/live\/sites\/urssaffr\/files\/outils-documentation\/guides\/Guide-TSA-2025.pdf\" target=\"_blank\" rel=\"noopener\">Taxe de Solidarit\u00e9 Additionnelle (TSA)<\/a> pour les contrats responsables<\/li>\n<li>le <a href=\"https:\/\/www.urssaf.fr\/accueil\/employeur\/cotisations\/liste-cotisations\/forfait-social.html\" target=\"_blank\" rel=\"noopener\">forfait social<\/a> pour les entreprises de plus de 11 salari\u00e9s<\/li>\n<li>la <a href=\"https:\/\/www.urssaf.fr\/accueil\/employeur\/cotisations\/liste-cotisations\/csg-crds.html\" target=\"_blank\" rel=\"noopener\">CSG-CRDS<\/a> sur la part patronale<\/li>\n<\/ul>\n<h3><strong>Les hausses annuelles in\u00e9vitables qui \u00e9rodent le pouvoir d&#8217;achat<\/strong><\/h3>\n<p>Les cotisations des mutuelles augmentent presque chaque ann\u00e9e. Ces augmentations s&#8217;expliquent par :<\/p>\n<ul>\n<li>le vieillissement de la population<\/li>\n<li>l&#8217;inflation m\u00e9dicale (consultation g\u00e9n\u00e9raliste \u00e0 30 \u20ac depuis d\u00e9cembre 2024)<\/li>\n<li>les transferts de charges de la S\u00e9curit\u00e9 sociale vers les compl\u00e9mentaires sant\u00e9 : la S\u00e9curit\u00e9 sociale d\u00e9cide de moins rembourser les soins. Pour compenser cette baisse de remboursement, les compl\u00e9mentaires augmentent la part qu&#8217;elles prennent en charge.<\/li>\n<\/ul>\n<p>L&#8217;\u00e9largissement du panier de soins remboursable (<a href=\"https:\/\/www.gerep.fr\/blog\/100-sante-ce-quil-faut-retenir\/\" target=\"_blank\" rel=\"noopener\">100 % Sant\u00e9<\/a>)<\/p>\n<p><strong>R\u00e9sultat<\/strong>, votre cotisation augmente chaque ann\u00e9e plus vite que votre salaire, grevant progressivement votre pouvoir d&#8217;achat.<\/p>\n<h3><strong>Le sentiment de \u00ab payer pour les autres \u00bb<\/strong><\/h3>\n<p>La mutualisation &#8211; principe fondateur &#8211; cr\u00e9e parfois une impression de d\u00e9s\u00e9quilibre, m\u00eame si elle constitue le c\u0153ur du syst\u00e8me. Un jeune salari\u00e9 c\u00e9libataire, en bonne sant\u00e9, qui utilise peu sa mutuelle, per\u00e7oit peu de gain imm\u00e9diat. \u00c0 l&#8217;inverse, un salari\u00e9 d&#8217;un foyer avec enfants percevra un b\u00e9n\u00e9fice bien sup\u00e9rieur.<\/p>\n<p>Cette mutualisation s&#8217;accompagne toutefois de trois biais principaux qui rench\u00e9rissent les co\u00fbts\u202f:<\/p>\n<ul>\n<li><strong>l&#8217;al\u00e9a moral :<\/strong> modification du comportement de l&#8217;assur\u00e9 qui, du fait de la souscription d&#8217;un contrat d&#8217;assurance, va a posteriori changer d&#8217;attitude et consommer plus que s&#8217;il \u00e9tait non couvert ;<\/li>\n<li><strong>l&#8217;antis\u00e9lection sur les options :<\/strong> comportement de l&#8217;assur\u00e9 qui choisit ou non de s&#8217;assurer en fonction de ses besoins ;<\/li>\n<li><strong>le biais inflationniste\u202f:<\/strong> la prise en charge par une mutuelle peut exercer une pression haussi\u00e8re sur les prix. D&#8217;o\u00f9 l&#8217;int\u00e9r\u00eat croissant des r\u00e9seaux de soins, o\u00f9 les tarifs sont n\u00e9goci\u00e9s et encadr\u00e9s&#8230;<\/li>\n<\/ul>\n<p>Pour les salari\u00e9s qui n\u00e9cessitent peu de soins, le <strong>rapport co\u00fbt\/b\u00e9n\u00e9fice peut sembler d\u00e9favorable<\/strong> \u00e0 court terme, m\u00eame si la protection reste essentielle pour les impr\u00e9vus.<\/p>\n<h2><strong>Conclusion<\/strong><\/h2>\n<p>Le lien entre pouvoir d&#8217;achat et mutuelle n&#8217;est pas aussi simple qu&#8217;il n&#8217;y para\u00eet. Loin d&#8217;\u00eatre un avantage imm\u00e9diat en esp\u00e8ces sonnantes et tr\u00e9buchantes, la <strong>mutuelle d&#8217;entreprise est avant tout une protection assurantielle collective<\/strong> qui co\u00fbte (cotisations, taxes, fiscalit\u00e9), mais qui prot\u00e8ge, parfois fortement \u00e0 des moments difficiles de la vie. La mutuelle pr\u00e9serve et m\u00eame am\u00e9liore le pouvoir d&#8217;achat en absorbant des d\u00e9penses importantes qui, sans elle, gr\u00e8veraient lourdement le budget des foyers concern\u00e9s.
